Can someone tell me what changing the baud rate for the upstream internets speed actually does?

If you have Ooma right after the modem and before everything else that uses the internet (router, computer, etc.), you can tell Ooma device how much upload bandwidth you have so that Ooma can reserve enough throughput for itself for phone conversations and then let the other traffic go through. This way Ooma can make sure sound quality is good even when lots of upload traffic is happening. You don't need to do this on the download side in general because download bandwidth is usually plenty.
