I have gotten several emails from Ooma support asking me to update my billing information. Each time I checked the box "use service address" and saved it. Apparently this does not work, because when I would go back to look at it, the billing information would be blank again. This time I entered the billing information manually and saved it. Hope it works this time.
Looks like billing address is good to me. Not sure what may have happened.
