I've received my LOA form and have typed in info but do not have ready access to a fax machine, especially if the number is long distance. Isn't there some latitude here for simply emailing a form which I have typed my info into? Receiving it via my email account on record, isn't this sufficient?

And can I just submit the header portion of my existing phone bill? I, too, have privacy concerns about the amount of info available in utility bills (no offense, Ooma). :)

I'd also like to retain my initial Ooma phone number as my secondary number, especially since I have already purchased Premier.

I've been very pleased with my new purchase but I need to get my port issue settled.

You can type your information directly into the LOA pdf file that you received via email. It must be opened in Adobe Reader to do this. Then save it to your hard disk and attach it to an email that you send to loa@ooma.com.
