I'm a new Ooma user. I don't think I will need any of the features offered by the Premier service, but it's still activated as part of my new user trial. At the end of the 90-day trial period, does it default to Ooma Basic or Ooma Premier? In other words, if I don't want the Premier service, do I have to opt out?

Unless you activated one of the older Core units, the default is automatic enrollment if you do nothing.
You can go into my.ooma.com anytime during the trial and opt out under services section. Opting out means you'll still have the same trial end date but you won't be automatically enrolled then.
