Using a Hub/Scout, made the porting request at the end of January 2010.

Ooma sent me several emails over the past few weeks notifying me that today (Feb 25) would be the porting day. Sure enough, this morning the landlines went dead to incoming calls. Four hours later, the numbers showed up on the Ooma boxes.

Everything appears to be normal and call quality is great. I will be saving $60/month.

My setup: Qwest DSL on a dry line to an Actiontec M1000 modem in bridging mode (in order to use M1000 as just a plain old DSL modem). Ethernet from the modem to a DLink DI-524 WiFi Router. Ooma Hub attached by ethernet to one of the available ports on the router. "Wall" port of Hub connected to "wall" plug of Scout. "Phone" port of Hub supplies one house line, "phone" port of Scout the other. Both lines tap into the existing house wiring. House wiring disconnected from the phone company outside at the box on the side of the house.
